There is a 3% service fee for the use of debit/credit cards.

This application must be fully completed and all questions must be answered. If you need assistance, please call the policy administrator, Outdoor Insurance Group toll free at: 1-888-683-7808. This is an application for insurance, not a Binder for Insurance.

This is a Master Policy,with an effective date of August 1, 2016 to August 1, 2017. The effective date of your policy will be on August 1, 2016 or the day after the postmark date on the envelope containing your application and premium payment if received after August 1, 2016 and be in effect until August 1, 2016. Premiums are not prorated if you apply after August 1, 2016. Premiums are 100% earned and there is no refund of premium if the policy cancelled midterm by club or landowner.
